Insurance & Payment

The following insurance plans are accepted:

  • BLUE CROSS/BLUE SHIELD – ANTHEM PPO
  • CIGNA PPO
  • AETNA PPO
  • MEDICARE
  • MAIL HANDLERS
  • UNITED HEALTHCARE PPO
  • OPTIMA PPO
  • NALC (National Association of Letter Carriers)
  • Any PPO plan

When you enter the office, please be sure to register at the receptionist’s window. Please advise her at this time of any changes in your address, insurance, marital status, etc. Also, if your insurance carrier required an authorization or referral, please provide her with that at the time you check in.

Financial Policy

Our practice is a provider with most major insurance plans. We process insurance billing as a courtesy for our patients. Please be aware that the costs not covered by insurance are your responsibility.

For services that are not covered by insurance, we request payment at the time of your appointment. Co-payments and co-insurances are due at the time services are rendered. Forms of payment accepted by the office are check, cash, insurance and the following credit cards:
